Optional Benefits
Full-time employees may elect the following benefits during their first 31 days of employment. If the employee chooses not to elect these benefits, they may be required to submit an Evidence of Insurability (EOI) application during summer enrollment.
- Optional Term Life Insurance: As an employee, you may enroll in one or two times your annual salary without proof of good health. You may also enroll in three or four times your annual salary, but proof of good health (EOI) will be required.
- Dependent Term Life Insurance: You may opt for life insurance coverage for spouse and eligible children. The coverage amount is $5,000 per dependent. If not elected during the first 31 days of employment, EOI is required.
- Accidental Death and Dismemberment (AD&D): You may choose coverage for employee or employee and spouse and/or children in the event of an accidental death. The minimum coverage for employees is $10,000 and the maximum is $200,000. In the event of the death of a dependent spouse, 50% of the employee's benefit is payable.
